Individuals, families and carers in the Mount Gambier area who are living with or affected by a life-limiting illness and who wish to receive our free In Home Hospice Care services will first need to go through an assessment interview.
Referrals may be made by a community or family member, doctor, the community palliative care team or other health services.
Once a referral has been received by the Hospice manager, the patient or carer will be contacted for a time to meet in their home environment to discuss the patient’s needs and suitability for volunteer support and to assess the home environment.
